CARE
Continuity Assessment Record and Evaluation. CARE is a standardized assessment instrument for quality improvement.
CCN
CMS Certification Number. The CCN is a unique identifier assigned by CMS to Medicare Part A Providers, and is also sometimes referred as a “Provider” or “OSCAR” number.
CMS
Centers for Medicare & Medicaid Services. CMS is the federal agency that administers medicare and medicaid services.

Electronic Health Records.
EHR Incentive Programs
The Electronic Health Records Incentive Programs. The Medicare and Medicaid EHR Incentive Programs will provide incentive payments to eligible professionals, eligible hospitals and critical access hospitals (CAHs) as they adopt, implement, upgrade or demonstrate meaningful use of certified EHR technology.

Internet-based PECOS
Internet-based Provider Enrollment Chain Ownership System (PECOS) is an on-line system by which physicians and other practitioners, and organization providers and suppliers may enroll, make a change to their Medicare enrollment, view their Medicare enrollment information, voluntarily withdraw from Medicare, or check the status of their web-submitted enrollment application.

National Provider Identifier.
NPPES
National Plan and Provider Enumeration System. National Plan and Provider Enumeration System. NPPES is the system that assigns NPIs, upon application, to health care providers and maintains each enumerated health care provider's record in NPPES.

PQRI
Physician Quality Reporting Initiative. PQRI is an annual quality reporting system, including an incentive payment for eligible professionals, established under the 2006 Tax Relief and Health Care Act
PS&R
Provider Statistical & Reimbursement. PS&R furnishes Medicare Providers with the data required to complete their annual cost report.
